“Just The Facts Ma’am”

Greetings my very gentle readers to anther weeks installment of the “Be Yourself, Often” blog found here at: http://www.joelwlamoure.com/id43.html . Ah, that time when we can sit back and appreciate the calm and soothing sense of nature and the chaos theory that abounds within Her.

This week we will take a look at one of my favourite movies, which is Dragnet. The modified one (of course) with Dan Aykroyd as the uber straightlaced, yet prone to impulsive decisions and actions Sgt. Joe Friday and Tom Hanks as his partner with a list of merits ad citations yet a flagrant disregard for the status quo. The dynamic and yet unusual duo that end up together through some cosmic mis-association in the LAPD. We all know the movie!

The Facts present us well and define us when they are filtered through the chaos of nature vs the amorphic and sometime ambiguous definitive lines that exist in a set culture, society or where there is a rigid pecking order. Quite intriguingly in my observations->  it seems across over 20 years of practice (11 in psychiatry), 140 journal articles, unknown OTN lectures and discussions, over 500 lectures, 3 continents and 60,000 hits to this website alone (plus 6000 on Academia.edu: www. http://uwo.academia.edu/JoelLamoure ) that the opposite is true.

Home is where the heart is. Not a place of straight lines, but one of amorphic curves. Per wit, embracing nature vs the man made entities that have their manipulation and unpredictabilities. Sometimes, one has to make a set stand in their Belief Systems and in their Creator to see that. These decisions are not easy, but do reflect how expectations can drive one insane. Totally, truly, clinically insane.

A wise person told me yesterday about anger, uncertainty and stress being like a balloon and how every event is like a puff into that balloon. And ways to distress and decompress that balloon…especially where there are factors that cause inherent impacts on the balloon to start with: like abuse, harassment, PTSD, anxiety, loss, grief to name a few. Interesting as I love writing and using metaphors in my sermons and writings.

“Just The Facts Ma’am” and knowing all the facts before making hasty decisions helps prevent inflation of the balloon and wiser minds to prevail. Step back and analyze what are the root causes and cause/effect and epidemiology and taxonomy of the situation Step back and analyze what are the root causes and cause/effect and epidemiology and taxonomy of the situation and prevent unneeded provocation and escalation. These are the facts of life to live. Peace, patience, silence, humility and serenity as I have stated many times (with varying success). I too fail, often.

Touching nature as it is meant to be in all its glorious, unrefined chaos is an appealing idea. To return to the roots and the Earth is to be yourself, often.

“All-Seeing I” OR “All-Seeking I”?

Greetings my gentle readers to another week's installment of the "Be Yourself, Often" blog at www.joelwlamoure.com . This is the time where we can sit back with the silence of our own mind and step back from the milieu of the week that was. Too often, we do not take the appropriate steps and moments of peacefulness and tranquility and rush through each thing in order to get to the next. Is really that not insanity?

Along the concepts of hope we appreciate that there are at least four dimensions that we need to explore in order to achieve optimal hope. The first is holistic health, second is outcome driven objectives, third is the fact that everything we do is personalized and the final aspect is that of empowerment.
I read once upon a time (for really, mustn't all stories, fairy tales and blogs start with that line?) that mankind is the creator of the straight line. Where my friends do straight lines exist in abundance in nature? Certainly at a more molecular level one might agree in the bonds between atoms, but really what we call a tree, a blade of grass etc. is NOT straight. Non-straight lines hurt our linear mind unless like a willow, we are willing to bend, adapt, adjust and do so without judgement. What do we strive for and what our objectives. For we know that Saul wandered until the scales were lifted from his eyes and then was enlightened in sight, spirit and soul.
Nothing is said about the commercialism or components of stuff infusing or suffusing him with the insight, but rather a flash of divinity and inspiration. Too often though that radar dish that is in our mind is shorted out and dulled by the promiscuity and curves of the seductress of life and stuff. We are lulled into a false fullness and complacency which curbs our enthusiasms and abilities to truly make a difference. Even worse, it makes it hard to see beyond the “All-Seeing I”. Even worse is that it becomes the “All-Seeking I” and there is nary a moment from friend or neighbor, friend or learnings but rather stuff, negativity and vitriol.
Too often we try to put the square peg into the round hole and try to make the darn thing fit and conform. Uh-uh, no way and ain't gunna happen. Cant we realize that we are equipped to aid, listen and share? To have all seeing and all-seeking but as and with the gifts and presents the world has to offer in the Present? Really, the miracles that abound?
The only way that we can make this equation work if we morph the peg to fit, either by accident or design. I have said it before that we are Model T Fords in a Ferrari society. Per wit, we have been transmogrified to "fit" the society allowance, rules, expectations and accountabilities. Remember the square peg? Thats got to hurt once you start cutting off the chunks that dont just conform beautifully (and I use that word in a very deliberate way) to the level of expectation. "Do this, do that, can't you read the signs" as the Five-Man Electrical Band would state in their song from the period of the age of Aquarius. Why can individuals not just respect the premises of intelligent concern and kindness for others and practice that? Resonate with the crystals that they are and feel the beauty and be able to express that inner joy? That would make some of the conformities more acceptable and shave off a little less of the peg... thus making life more rewarding and fulfilling. What is important, respecting others, caring, a few true friendships, a soft word and the scents of ambrosia.
These are the things of the amorphic. Sorry people, we are so much more than a job, title, position and user of oxygen on this earth. We need to go beyond the tangible and look to the special, secret moments and quiet voice that exists within all of us. To express ourselves as a flower expresses itself in what we do, how we function and how we dress. To judge these freedoms of expression that do no harm to others is well, narcissistic and egocentric. Sorry... you are selfish. "Judge not, lest ye be judged". Dog eat dog baby.
Does it have to be this way? Nope. Does everything have to be rationalized to death? Nope. Does everything have a rational answer? Nope. And by looking at the irrational and the spur of the moment, the winds of war... the greatest insights, opportunities, gifts and flames alight. Keep your eye open for those Doors of Fate that arise in the midst of the darkness, not focusing on the fact your life sucks that you are in a period of darkness and that you DESERVE (darn it) the light and gifts.
You know my friends, you might... just might find a square hole. How? Express yourself with gratitude, humility and with hope, faith and belief and Be yourself, often.

Thanks for ALL blessings on Thanksgiving

Small cheer and great welcome make a merry feast. William Shakespeare.
We give thanks for unknown blessings already on their way. Ritual Chant, religion unknown.
This is the time my friends for Thanksgiving in Canada. To my American friends and colleagues, y'all have to wait for a few more weeks. However the sentiments hold true day to day, so please don't stop reading, ok? Maybe it is and should be a single day event, and can be conceptualized much like religion.
How can that be? That is being that often with religion we “do our duty” for an hour a week and then as soon as we get out of the parking lot its time to vent and bring back out the person that we are. No piousness, but hypocrisy. Sad isnt it? So instead of giving thanks for 1 day of the year, here is that challenge, that being to appreciate the smallest things of life at point of need. There truly are no atheists in the foxholes. Would be such a horrible thing to biatch, moan and whine your way through life and only see the positives when tragedy or the end of days tends towards rolling the credits of the movie that was your life.
Thanksgiving this year, as it should be every year is a time for reflective, meditation and appreciation. That is an appreciation of person, animal and crystal…all living things that co-exist on this rolling blue marble before its too late. Perhaps more of a spiritual perspective would help give us all some clarity.
Oh my friends, it is so easy to give Thanks for the good things in life and the plentiful bounties that are there and the ready access to so many amazing and incredible things and gifts that we have. For the merry feasts and family and loved ones. For the opening of hockey season (if it ever comes) and the roof over our head and all of the plusses on the positive side of the ledger of life. But there is so much more than that to give thanks for, in the life, respiration, colours, sights, sounds and all the blessings and interactions that we are able to sense and touch. A time for blessings and peace if we so choose that if handled correctly, allows our true angelic wings to spread that will ripple out benefits and healings in concentric circles. Not negatives, only positives…yet sadly too many choose the hard route of only focusing on the negatives. That is that there is anger, resentments, feelings of being put down, left out etc etc. L:ife isnt fair to YOU when you are full of negatives. Waaah you.
But this is also a time to give Thanks for the challenges in life, the red ink in the ledger in life that we have experienced and continue to experience. The ledger of life will balance out at the end when the summative tally is assessed and the call to accounting is made and the ultimate withdrawal. But how many actually look at how they handle the challenges in life? How do you deal with these "unknown blessings?" Can you believe I could call a challenge an unknown blessing? The blasphemy of it all! If you think or are thinking this, you my friend are losing out big time. Missing life, opportunities and frankly have a little too much self-entitlement for my liking. Good or bad, richer or poorer, sickness in health we should always be learning, always taking stock.
It takes a fortitude and culture of self courage and self-drive to tackle the negatives from a learning perspective. If you want any supportive evidence my friends, look in the mirror and do a bit of self-reflection and mindfulness on the moment. You are where you are at this point of time of reading this because of all the goods in your life, AND the challenges. If you get stuck in a rut of challenges, you may need to reflect on why you keep getting the same negative outcomes. <bit of a hint here.... try something different!>  You dont have to LIKE the negatives (consider masochism and its linked psychopathologies via root cause analysis) but you DO have the power to decide HOW they affect you and What you are going to do about them!
The expression goes that you dont appreciate something until its gone. Thats why you need to live the moment, seize the day and please take nothing for granted. Whether it be a voice, hug, sensation, feeling, sight or smell... never take it for granted. Life is not designed to be a fight, and uphill battle that we lose, but a gift.... a present. Dont lose out.
To all the happiness and sadness, wins and challenges and positives and negatives.... thank you. Love is an intelligent concern for others. I learned that a decade ago and one of the critical learnings of my life.
Be yourself, often.

August 22, 2012

In 2001 there was establishment of a UNESCO Chair designation in medical bioethics resulting from published research under Professor Carmi from 1996 and 2001. This research centered on the specific field of the provision and outcomes of medical bioethics education. Secondary to this research, a UNESCO International Chair position was created for a UNESCO Chair in Medical Bioethics. An international network was created from interested and supporting Universities across the world, with each of the 29 signatory countries being designated a UNESCO Medical Education Bioethics designated University, with a corresponding National Network and Chief/Lead for their country. In 2009, Canada became a signatory to this initiative. Western University enjoys collaboration with the other Bioethics Universities including Harvard in the United States, Tokyo Medical University in Japan and the University of Haifa in Israel to name a few.

These networks have been tasked with determining the status of medical education deliverables in bioethics across the country, including quality, placement, sustainability and robustness within the medical curriculum.

The Canadian Network at August 22, 2012 has been awarded by the UNESCO Chair to the supervision of Dr. Joel Lamoure, a critical care pharmacist and Associate Professor in the Department of Psychiatry. Dr. Lamoure also serves as a scientist with Lawson Health Research Institute and surveyor with Accreditation Canada. Joel was licensed as a pharmacist in 1991 and completed his Doctorate in Divinity in 2011.

A revised cross-Canadian panel and committee will be convened to address and go the next level of questions that have been posed by the UNESCO Bioethics Chair to support the Charter.

The aim of the Network is to ensure a robust and evidence based approach to the provision of medical bioethics within the medical curriculum. In a sharing, international network format we aim to determine the level of current education, identify the needs for bioethics education and train the educators through adult based learning educational strategies.

UNESCO has noted the emergence of the need for bioethics at grass roots, educational level that is continuous, consistent and mirrored through education, practice and mentoring. Their statements surrounding the current challenges are as follows*:

“There is an emerging need for introduction of teaching medical ethics as a consequence of several social and scientific processes:

•  Health-care consumers emphasize nowadays not only the need for health but the need for quality of life. Patients expect professionalism, effectiveness and quality, along with empathy, reliability and devotion.
    • Health-care providers are detached from traditional concepts of idealistic medicine, adopting a contractual, consumer paradigm.
    • Medical technology has created new dilemmas (e.g. procreation, euthanasia, intensive care, medical genetics, bio-technology), while at the same time causing previous ethical resolutions to become obsolete (e.g. definition of death, family composition).
    • Specialization and sub-specialization in medicine have encouraged technicality at the expense of patient-physician relationship and communication skills, thus creating a growing gap between physicians and their patients, and between medicine and society at large.
    • Growing social concern, suspicion and demand for closer inspection on medical activities is filling this gap. The demand is materialized in the form of ample litigation, increased health-related legislation and formulation of international declarations, conventions, charters etc., creating new ethical and legal frameworks and new obligations for the practicing physician.
    • Resource allocation in face of growing monetary constraints creates a substantial effect on the everyday practice of medicine.
    • The need to adhere to ethical norms in scientific research and experimentation (human cloning, pharmacology etc.) remains a constant challenge”

We would like to thank the previous Canadian Network members for their immense efforts to date since 2009 to bring Canada to the table in Bioethics as it relates to education and setting the stage for the next exciting stage of development. Being that education based in CANMEDS is a life long role, we look forward to creating new Canadian linkages to move the medical education bioethics across the scope, specialties and lifespan of clinicians.
